This is the first brooch Serena/Usagi receives.
It is given as a gift from Luna.
In the American version, Serena shouts "Moon
Prism Power!" to activate the transformation
sequence of the brooch.
In the original Japanese, Anime Usagi shouts "Moon
Prism Power Make-Up!"
There
is nothing inside this brooch. This brooch will break in episode
51 of 200. Sailor Moon will lose her transformation and go back
to her human self. Then Queen Serenity will appear with the Silver
Crystal.
The Silver Crystal then merges to form Sailor
Moon's new transformation brooch. Queen Serenity also warns
her daughter that the Silver Crystal is no longer at its full
strength (since much of its energy was depleated battling Queen
Beryl), but one day Sailor Moon might be able to use its full
energy again if she wishes to save the people she loves.

This is the transformation brooch for the 2nd season
of Sailor Moon - generally called Sailor R. This brooch appears
in episode 51 of 200.
In the American version, Serena shouts "Moon
Crystal Power!" and transforms.
In the Japanese version, Usagi shouts "Moon
Crystal Power, Make-Up!" and transforms.
In the Japanese version, Sailor Moon shouts "Moon
Crystal Power, Refresh" to heal and purify the Four
Phantom Sisters.

This
is the transformation brooch from Season 3. It is revealed at
the same time as the Moon Heart Scepter to Sailor Moon and Prince
Darien.
Usagi shouts "Moon
Cosmic Power, Make-Up!" With this, she
transforms into Super Sailor Moon.
This
brooch also gave Sailor Moon a double transformation when the
Holy Grail was discovered. With the words "Moon
Crises, Make-Up!" Super Sailor Moon was able
to transform into Super Super Sailor Moon for a brief period of
time. Then her energy would be exhausted and she would transform
back into Super Sailor Moon.
In the Fourth Season - Super Sailor Moon 
would
no longer have the Holy Grail because it was destroyed by Pharoh
Nine and Mistress Nine, during the ending battle of Sailor S Series.
Therefore she would have to resort to the help of Pegasus to transform
into Super Super Sailor Moon. A transformation that will
also only last temporarily before she returns to her Super Sailor
Moon state.
Super
Moon Crises Power (US) is used in the Super S Sailor Moon
movie for the equivalent of Moon Cosmic Dreamaction (Jap). This
is a combo attack with both Sailor Mini Moon and Super Sailor
Moon.
Also in the 4th season, Sailor Moon's Moon Heart
Scepter would be the Moon Sword for the period of time that Pegasus
shares his power with Super Super Sailor Moon.

This is the last and final transformation sequence.
At the beginning of the 5th season, Sailor Moon would battle
Queen Nehelenia again. In Sailor Saturn's dream Sailor
Moon was able to transform into her final stage - Eternal Sailor
Moon.
At the beginning, this could only be achieved when
all the Sailor Scouts combined their energies into Sailor Moon.
But in Sailor Moon's final attempt to save Mamoru from Queen Nehenelia,
Usagi's tears transformed her brooch into the brooch you see above,
without anyone's help. After that miracle, all Usagi has
to do is shout "Moon
Eternal, Make-Up!" to reach her last and final ultimate
transformation - ETERNAL SAILOR MOON!
